New Passport Document Requirements
- Birth Certificate
A certified copy of your birth certificate: You may not use hospital records as a substitute. - A passport photo
One recent passport photo. Please make sure the photo meets the U.S. passport photo requirements. - New Passport Application
Complete a NEW PASSPORT APPLICATION. Make an extra copy for Atlas Passport Service, and DO NOT SIGN THE APPLICATION YET. - Fees
A cashier’s check or money order in the amount of $211.36 ($181.36 for minors) made payable to “US Department of State.” (Note: The applicants may also apply for a “PASSPORT CARD.” If you are applying for both documents in the same application, mark the corresponding box on your application and add an additional $30 to the filing fees. Please note: Post offices (Acceptance Facilities) and Courthouses will charge $35 for a local processing fee. This is standard protocol for these facilities and a part of the federal passport processing fees. - Identification
Include a copy of the front and back of your current and valid driver’s license or State ID. - Proof of Travel
Include a copy of the airline ticket or travel itinerary issued on a travel agent or travel portal letterhead. Businesses may submit a business letter addressed to the Passport Agency requesting expedited service and a valid reason for the request. If you have had a name change, include a certified copy of a marriage certificate or certified copy of court order/decree. - Authorization Forms
Complete and sign two original authorization forms (MUST USE BLACK PEN). These forms are required by law to allow Atlas Passport Service to submit your documents and to receive the issued passport from the Passport Agency and must be included in your packet. Click here for Authorization Form.

B. Take above documents to local clerk of the court, or a Passport Agent at a Post Office:
Sign the application in front of the clerk/agent. Ask the clerk/Passport Agent to endorse the documents (applicable items 1 through 8), seal them in an official envelope, affix his/her seal on the envelope, and mark it to read: "TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY." Do not leave the documents with the clerk/Passport Agent. Ask that the package be released to you for hand-carry-expedited processing.
C. Pay for services
$389.90 processing fee.
$69.00 ($85.00 for off-shore and Saturday deliveries) for Federal Express return of your new passport.

- Prices are per person per passport.
- We assist clients to obtain expedited passports by using our submission spots allocated by the Passport Agency. These spots have monetary value and once used, or expired, or when you fail to complete the return of your sealed package within agreed upon time frame for any reason, they are gone and the service fee cannot be refunded. The service fee is not transferable and submission spots cannot be stored/postponed for later use.
- Due to the indivisible nature of this unique service and resources engaged, no refunds can or will be issued.
- You can also use your old expired passport for proof of citizenship. If your most recent passport (issued within the past 10 years) is lost, misplaced or stolen, then, in addition to the passport application you'll need to also complete an application "Statement Regarding Lost or Stolen Passport" (DSP-64), and include it with your package.
- You'll need to factor-in the time it takes to send the passport for processing and the time it takes the issued passport to be sent back to you. Ordinarily and depending on the days we are allowed to submit passport applications to the Passport Agency this process may take up to ten (10) business days at our end.
- Allow yourself sufficient cushion time! Expedited passport processing service is time-sensitive and unforeseen issues can cause delays. The entire risk burden of accurate information on your application and other forms including data entry, writing of your full name, misspellings, marking or not marking the proper boxes, correct Social Security No., dates, etc. and resulting delays, are on you. Please know that all decisions including acceptance, denial, rejection, suspension, and additional supporting document(s) requests notices, or approval and issuance of passports, are solely and unilaterally made by the Passport Agency. We are not involved in the decision-making or passport issuance process. Nor, the Government will entertain any out-of-guidelines submission requests through APVS. Beyond timely submission of application packages and subsequent, or timely communication of government notices to our clients, APVS, its employees, agents and affiliates, in no way, expressly or impliedly, make, or can make, any representations or guarantees regarding acceptance of documents, issuance of passports, amendments/modifications, or processing times, beyond or in addition to the information provided here. We are not responsible for delays caused by administrative errors/omissions, computer breakdowns, unexpected Agency closures/meetings, additional documents that the Passport Agency may require, misspelling of names, wrong dates, rejection/denial/suspension of applications; or for delays due to omissions, or negligence of Passport Acceptance Facility agent(s) i.e at U.S. Postal Service facilities or city halls when they don't do their job properly, or ignore/refuse to follow guidelines. Please submit a completed, signed, and dated PASSPORT RENEWAL APPLICATION (USE BLACK PEN TO SIGN AND DATE). Please note: The Passport Agency requires that we submit executed/signed applications within FIVE (5 days) of Signing. Please keep this time frame in mind when submitting your application. - Proof of Travel
Include a copy of the airline ticket or travel itinerary issued on a travel agent letterhead or through the online portals. Businesses may submit a business letter addressed to the Passport Agency requesting expedited service and a valid reason for the request. Submit a check or money order for $211.36 payable to "U.S. Department of State." (Note: Applicants may also apply for a “PASSPORT CARD.” You may apply for both passport renewal and the passport card at the time of your renewal but be sure to mark the corresponding box on your application and submit an additional $30 for your Passport Card. - Name Changes

- Please note that the passport renewal processing necessitates shipping important and mostly difficult-to-replace original documents, including passports, IDs, certificates, Etc. to the Passport Agency Facilities. We are not responsible for delays caused due to additional document requests, tardiness in adjudicating/reviewing your case, rejection of your case, or other administrative delays while the Passport Agency is handling your case. The processing times that we tell you are mere "for-reference-only" good faith approximations based on the available information on similar processes to date and are not intended for detrimental reliance on your part to change your position in any way until you receive the renewed Passport. Especially since they are subject to change by the passport Agency without any prior notice. Please examine the completed/prepared documents to be sure of the preferred spelling and accurate data or for inadvertent typing errors before signing. - Since your documents have to be shipped to a Post Box address, we use the traceable "Priority Overnight" from the Post Office to send the current Passport and the supporting documents to the PO Box designated for us by the Passport Agency. We are not responsible for mis-deliveries [or even loss or destruction of documents] by the USPS or the Passport Agency. In case of a possible loss and the necessity of placing a stop payment on the payment check, you are responsible for the associated stop payment fees for your check and for any check we have issued on your behalf. Please note that our service ends once we ship your documents. We are not responsible for curing possible issues associated with issuing your Passport that may arise at the processing facility. Further, we are not responsible for securing an appointment for you to see a federal officer and submit additional forms and documents. Child Passport
All Minors MUST appear in person, with both parents with valid identifications (and photocopies of front and back of their IDs) for signing at the Post Office. If one parent cannot appear in person, that non-appearing parent must complete a notarized “Passport Consent Form” along with a copy of the front and back of his/her ID. The appearing parent will present the consent form at the time of signing in front of the federal agent. The consent form MUST be completed using a black pen in handwriting, and a copy of ID used to Notarize the signature MUST be attached to it. You may obtain a “Passport Consent Form” by clicking here.

We accept all major credit cards (PLEASE NOTE: Since federal filing fees must be paid by check or money order, except for mail-in New Passport requests, if you choose to pay the federal filing fees using a credit card, additional service/finance fee of $35.00 will be added to the federal fees so that we may issue a company check for you. Please print, fill-in your credit card information, complete, initial and sign this payment authorization form personal data sheet, place it on top of your completed application/package and mail or overnight express your package to:

Please Read:
- Prices are per person per passport.
- We assist clients to obtain expedited second passports by using our daily submission slots allocated by the Passport Agency. These slots have monetary value and once used, or expired, or when you fail to complete the return of your sealed package within agreed upon time frame for any reason, they are gone and service fee cannot be refunded. The service fee is not transferable and submission slots cannot be stored/postponed for later use.
- Due to the indivisible nature of this unique service and resources engaged, no refunds can or will be issued.
- You must submit your current valid passport to the Passport Agency along with the rest of the paperwork/documents.
- You'll need to factor-in the time it takes to send the passport for processing and the time it takes the issued passport to be sent back to you. In reality this process can take 3-4 days.
- Allow yourself sufficient cushion time! Expedited passport processing service is time-sensitive and unforeseen issues can cause delays. The entire risk burden of accurate information on your application and other forms including data entry, writing of your full name, misspellings, marking or not marking the proper boxes, correct Social Security No., dates, etc. and resulting delays, are on you. Please know that all decisions including acceptance, denial, rejection, suspension, and additional supporting document(s) requests notices, or approval and issuance of passports, are solely and unilaterally made by the Passport Agency. We are not involved in decision-making or passport issuance process. Nor, the Government will entertain any out-of-guidelines submission requests through APS. Beyond timely submission of application packages and subsequent pick up of issued passports, or timely communication of government notices to our clients, APS, its employees, agents and affiliates, in no way, expressly or impliedly, make, or can make, any representations or guarantees regarding acceptance of documents, issuance of passports, amendments/modifications, or processing times, beyond or in addition to the information provided here. We are not responsible for delays caused by administrative errors/omissions, computer breakdowns, unexpected Agency closures/meetings, additional documents that the Passport Agency may require, misspelling of names, wrong dates, rejection/denial/suspension of applications; or for delays due to omissions, or negligence of Passport Acceptance Facility agents i.e. at U.S. Postal Service facilities when they don't do their job properly, or ignore/refuse to follow guidelines. If your documents are in order, and absent any other issues, however, the passport should be issued promptly.
